fistula is abnormal connection of pus Carrying non healing tube like wound between perianal area to Anus inside. 

 

 

how fistula forms?

 

when hard stool rubs over soft inner side of anus, wound is formed inside anus. This wound is good habitat for bacteria to enter inside mucosal layer. These bacteria creates one hard layer around tube which does not allow medicines to enter in tube. And hence bacterial habitat grows well inside tube.
