Difference between Injection
Treatment for Fissure and Piles
While practicing as a proctologist we got many enquiries
regarding injection treatment for piles. Most of them have heard about this
injection treatment. Or some other doctors may have miss guided them that
injection treatment is available with them. To clarify about this we have a
further explanation.
First diagnosis of the patient must be confirm to give
injection treatment.
1) If the diagnosis is fissure in ano then the injection
treatment is Boutox injection.
2) and if the diagnosis is first or second degree of piles
only, then the injection treatment means sclerotherapy.
Boutox injection treatment- Botulinum toxin is the injection
used for muscular spasm disorders. When there are severe spasm of muscles then
this injection is injected inside the muscle to get relief from spasm. exactly
in fissure in ano there is a spasm of internal sphincter which make the anal
space narrow. While passing the stool, space is not enough and it rubs over the
mucosa and while coming out linear ulcer formed at mucosa. when this injection
is injected in internal sphincter it relieves some sort of spasm of internal
sphincter. But this may form abscess at anal region. Some patients may not get
relief from this injection. For them doctor has to repeat the same injection.
And it is little painful. The cost of injection is around 12000. And if
constipation and hard stool continues then it will reoccurre. Due to its less
effectiveness and complication it is not advisable. Some consultant may perform
another procedure by giving injection around anus as local anaesthesia. Patient
feels that it is an injection treatment.
2) Sclerotherapy for bleeding piles- some patients complaint
of bleeding per rectum only. They don't have much pain at the time of
defecation. Means they are not suffering from fissure in ano and pathology is
only first or second degree hemorrhoids. Injection askerol is injected
submucosally to introduce fibrosus
between venous plexus, pile mass and mucosa. By that bleeding get arrest. this
type of treatment can be used for the patient who are not able to get operate
due to there pre existing disease condition like heart disease, chronic renal
failure etc.
so before going for treatment, one must ask your doctor for
exact right of treatment.
